CHSI is a private, non-profit organization established in 1973 to serve the health needs of migrant & seasonal farmworkers and their family members. Today, CHSI has expanded services and provides mental & physical healthcare to all individuals in our communities. Our services are provided on a more affordable sliding fee scale to those who are under-insured or un-insured. We also accept ND and MN Medicaid, Medicare, and most private insurance plans. We currently operate four nurse-managed clinics, two mobile medical units, and two victim advocacy program centers in Minnesota & North Dakota.


We are on the search for a full-time Behavioral Health Navigator to join our Rochester, MN team. In a Behavioral Health Navigator role at CHSI, you would coordinate and provide access to community-based specialty mental and behavioral services; facilitate communication among all providers; empower patients to be active participants in their healthcare. These services may include:

  • Maintains confidentiality with all clients and staff.
  • Register and update patient information and set up appointments.
  • Participate in community outreach events.
  • Responsible for all components of the registration process.
  • Knowledgeable about community services in his/her area, serving as a resource person for staff and community providers.
  • Interpret for staff, patients, and community service providers when necessary.
  • Encourage a stable and regular relationship with a primary care provider
  • Provide education about medical conditions and recovery strategies
  • Facilitate communication with health care providers
  • Participate in site-based staff meetings and interdisciplinary project teams
  • Responsibility to comply with all pertinent regulations under HIPAA
  • Ability to establish rapport quickly and to communicate effectively with patients, their family members, and community providers
  • Ability to function effectively as a member of an intra-professional team that includes behavioral health and primary care providers, patients, and family members
  • Excellent knowledge of community resources for mental health, behavioral health, and other social service needs
  • Assist with creation and implementation of integrated care plans, ensuring access to an array of linked services, and the exchange of information among patients, family members, and providers


In addition to being a wonderful team member and coordinator, we are seeking an individual with the below qualifications:


  • In an eight hour workday, the employee must be able to: bend and crouch occasionally,
    sit continuously and stand or walk frequently.
  • Employee must be able to lift up to 50 pounds occasionally.
  • Employee must be able to use hands/arms, feet/legs for repetitive fine to large motor skill


  • High school graduate or G.E.D. equivalent
  • Fluent in both English and Spanish
  • Strong interpersonal skills
  • Can manage multiple projects and is self-motivated
  • Maintain confidentiality with all clients and staff
  • Proficient with Microsoft Office Suite
  • 1-2 years of experience of community behavioral health system preferred
  • Knowledge of community health resources and how to help clients take advantage of programs that will improve their overall health status.
